Peripheral Nerve Field Stimulation for Chronic Abdominal Pain

BACKGROUND AND AIMS
Abdominal pain is the leading gastrointestinal complaint for outpatient visits, comprising 12.2 million visits per year and is also the most frequently cited chief complaint in hospital emergency departments, accounting for 7% of all emergency department visits. The etiology of pain in a patient with chronic abdominal pain is complex and can be difficult to control. We report successful treatment of chronic abdominal pain developed after multiple abdominal surgeries with peripheral nerve field stimulation (PNFS).
MATERIALS AND METHODS
A 38-year-old female presented with four year history of multiple abdominal surgeries and chronic right upper abdominal pain. Conservative medication management, trigger point injections, celiac plexus block, intercostal and epidural injections gave the patient no relief. She underwent successful trial of subcutaneously placed over right upper abdominal wall of two 4-electrode leads (Medtronic Inc., Minneapolis, MN). During PNFS trial patient reported greater than 75% improvement in pain and two weeks later underwent placement of permanent leads and RestoreULTRA rechargeable generator.
RESULTS
After final implantation all patients reported significant pain relief (>90% reduction in VAS) at 12 months and used stimulator 24-hours per day. Patient discontinued use of opioid and non-opioid pain medications. She was able to resume her occupational duties.

CONCLUSION
PNFS is a treatment option for chronic abdominal pain in patients who have exhausted conservative therapies and more conventional interventional pain techniques. Neuromodulation techniques including PNFS can result in adequate pain control and lead to functional recovery.
